[Adta] DMT and Special Needs Children

Is anyone using dance/movement therapy in a dance studio setting. I have an opportunity to be starting a creative dance program for children with special needs in the N. Dallas area (Frisco). I want to create a form and am wondering what others use to gather pertinent info. I realize that special needs is very general and broad, hence why I am reaching out to the list serve in hopes that someone can share their knowledge, or pass on any assessment/registration type forms they may have.
